To improve the provision of medical care in the Armed Forces of Ukraine, the paper medical card will be replaced with an electronic one. How should it work and when to expect its implementation
The Ministry of Defense will introduce an electronic Primary Medical Card, which will replace the outdated «Form 100».

The form of the electronic Primary Medical Card was approved by the Order of the Ministry of Defense No. 879 dated 31.12.2024 on approval of the form of primary accounting documentation «Primary Medical Card». The document will now be maintained electronically in the information and communication system «Medical Information System of the Armed Forces of Ukraine», the ministry reported. However, the instructions note that if there is no technical possibility to fill out the form in the ICS MIS AFU, it is allowed to maintain it in paper form.
The form is a medical document confirming the provision of medical care at the pre-hospital stage and is used to generate reports. It is filled out in the event of injury (wound), illness or damage for injured servicemen of the Armed Forces of Ukraine and other institutions.
«This is an important step that creates an opportunity to collect and analyze medical data, on the basis of which decisions will be made to improve the provision of medical care at the pre-hospital stage,» said Nikita Nikitenko, State Secretary of the Ministry of Defense, who oversees, in particular, the health care sector at the ministry.
Form of primary accounting documentation No. 001/o «Primary medical card»
Form No. 001/o «Primary Medical Card» must be filled out by medical professionals in an outpatient clinic or hospital. The form is also printed if a patient during medical evacuation is transferred from the pre-hospital stage of medical care (role 1 or role 2) to a civilian healthcare institution. The document is stored for 25 years.
The form contains general information about the victim, previously provided assistance, injury or illness, as well as the amount of assistance provided in a medical facility. In addition, the document includes data on the patient’s vital signs, diagnostics and resuscitation or other interventions. In the event of death, information about the time, date and circumstances is also entered in the document. More information about filling out the form can be found in the Instructions .
The implementation of the order to create an electronic Primary Medical Card is also the basis for the operation of the combat trauma registry, which the Ministry of Defense is working on launching. The data from such a registry, in particular, is used to form an evidence base for treatment protocols in NATO countries.
As noted by the Minister of Defense of Ukraine Rustem Umerov, the Ministry of Defense and institutions are obliged to introduce the form within three months from the date the order comes into force.
Within the same period, the technical possibility of maintaining this card using the Medical Information System of the Armed Forces of Ukraine must be ensured.
